Checklist for a survey design
Use this Checklist to guide you in developing a well-designed survey. You can download the checklist for your later use.
Planning Phase
Clearly define the research question.
Establish specific aims and objectives.
Identify key concepts and define them using standardised definitions.
Determine the target population, contactable population, and sampling frame.
Consider the recall period and relevance for retrospective questions.
Consider ethical implications and prepare a Participant Information Sheet (PIS).
Develop a research data management plan.
Design Phase
Select an appropriate survey method (e.g., online, phone, in-person).
Ensure the tool/platform supports needed customisations (e.g., skip logic, branding).
Apply CSU branding and accessibility guidelines.
Use simple, clear, non-biased language (avoid jargon, ambiguity, acronyms).
Choose appropriate question types (open, closed, scales, rankings).
Avoid leading, double-barrelled, absolute, and assumptive questions.
Group questions logically and apply sequencing principles (e.g., non-sensitive to sensitive).
Limit mandatory and open-ended questions to reduce fatigue.
Estimate and manage survey length (aim for <10 minutes ideally).
Testing Phase
Pilot test the survey with target users or colleagues.
Assess readability, clarity, response time, and logic functionality.
Test branching, skip logic, and device compatibility.
Revised based on feedback and identified dropout points.
Distribution and Analysis
Use appropriate tools for data collection (e.g., Qualtrics, REDCap, MS Forms).
Ensure survey access (e.g., QR code/link working).
Match analysis tools to data types (SPSS for quantitative, NVivo for qualitative).
Document incomplete/partial response handling.
Plan for follow-up (reminders, thank-you message, optional interviews).
